Looks like a problem with backlash. You've already checked the set screw. I'd take a look at everything touching the lead screw: all set screws, the ball nut or clamping block, any couplers, etc. When the CNC changes directions on that axis, the gantry isn't responding right away.

TAW is on the right track. It may be as simple as correcting the tilt on your router once you've figured out how bad it is. Bill Z has a good description of how to measure and correct that here:

The Z setting will depend on your project file. You can start from either, but I prefer to start at the top of the material and set the depth incrementally deeper, as needed, to cut through your piece. I do something similar to this for vinyl cutting.

What is a dog knife?

I've used a drag knife to cut vinyl sheets. Donek Tools has a drag knife that's supposed to be very good, but I made my own. I'm not sure what a dog knife is, but I'm interested.

The controller box can turn on and off the router, but you need to set the router speed yourself.
