HD4 Failures during 3D Finish

While running a 3D finish toolpath the bit dips down into the wood returns to the correct depth and continues on. This problem just started happening. I've disconnected all of the controller connections and retightened them, opened the controller and found no loose connections on the terminal block. I noticed at the end of the single fail run the pendant showed 15 ignored lines, at the end of the double, 27 lines ignored. I panic stopped the total fail so I have no idea how many lines were ignored.
Any ideas if the ignored lines are connected to the problem? What causes lines to be ignored by the pendant/controller? I've noticed that all good runs have only one ignored line.

Thanx Bob.
I use a zero plane should it be set at the depth of the dish (0.04in) instead of 0.00?
The ignored lines in the .tap file still worry me.
