Router runs off path

I have had a CNC Shark for about 12 years. It doesn't happen all of the time but sometimes in the middle of the run it will jog off line and mess the whole piece up. When I stop and try and move x, y, or z they barely move. If I shut down Mach 3 and restart a few times it seems to work. Is this a hardware or software problem?
